Enhanced Primary Care (EPC) Plans

Have you recently seen your GP regarding a chronic medical condition and received a referral form for Individual Allied Health Services under Medicare? Does your referral include services provided by an Osteopath at Live Well Health Centre?
 
If so, you may be eligible for a Medicare rebate on up to 5 Osteopathic treatments within the calendar year.
 

Our process at Live Well Health Centre

• Upon receiving your Enhanced Primary Care (EPC) Plan from your GP, you can book an appointment with one of our Osteopaths at your earliest convenience.
• If all the information is provided on your form, simply give us a call on (03) 9363 2961 or book online, mentioning that you have a Medicare referral.
• Your GP will have provided you with a referral form which we require you to present at, or prior to your appointment, either in paper form or via email.
 
Your care plan form should have the details in order for us to process your rebate
  • Name of the clinic 
  • Date of request
  • Number of visits (maximum of 5 within the calender year)
  • Item code for Osteopathic services (10966)
• On the day of your appointment, you will be required to pay for the consultation in full.
 
• Following this, we will process your Medicare rebate for you, which will appear in the bank account registered to your Medicare account within 24 hours.
Fees:
  • $130 for Initial Appointment
  • $100 for Standard Follow up Appointment
The current Medicare rebate is $60.35.
 

The Live Well Difference!

At Live Well Health Centre, our Osteopaths will send an initial report to the referring GP, informing them that you have commenced treatment. Throughout the treatment process, our Osteopaths will continue to inform the GP of your progress in writing, and liaise with them in regards to ongoing care and management.

Can I claim both Medicare and Private Health Insurance?

Medicare rebates and Private Health Insurance benefits cannot both be claimed for 
these services
